Waiver, Release of Liability & Parental Consent (Minors)

This Waiver, Release of Liability, and Parental Consent (“Waiver”) applies to all participants under the age of 18 who register for or participate in programs, leagues, events, training, or activities sanctioned by the Niagara Soccer Association (NSA).

By registering a minor, the parent or legal guardian confirms that they have read, understood, and agreed to the terms below.

1. Participant Information

This Waiver applies to:

Players under 18 years of age
Training sessions, matches, tournaments, camps, and related activities
Activities conducted on or off NSA-approved facilities

2. Acknowledgement of Risk

I, the parent or legal guardian of the registered minor, acknowledge and understand that participation in soccer involves inherent risks, including but not limited to:

Physical injury (sprains, fractures, concussion, cuts, bruises)
Collision with players, officials, or equipment
Environmental risks (weather, field conditions)
Transportation to and from activities (unless provided directly by NSA)

I understand that these risks may occur even when all safety rules are followed.

3. Assumption of Risk

I voluntarily assume all risks, known and unknown, associated with my child’s participation in NSA activities.

I confirm that my child is physically fit and medically able to participate, and that any relevant medical conditions have been disclosed during registration.

4. Release of Liability

To the fullest extent permitted by law, I hereby release and hold harmless:

Niagara Soccer Association
Its directors, officers, employees, volunteers, officials, referees
Affiliated clubs, teams, coaches, sponsors, and facility owners

from any and all claims, demands, actions, damages, or liabilities arising from my child’s participation, except in cases of gross negligence or willful misconduct.

5. Medical Consent & Emergency Treatment

I authorize NSA and its representatives to:

Obtain emergency medical treatment for my child if necessary
Share relevant medical information with emergency responders

I understand that all medical costs are my responsibility.

6. Insurance Acknowledgement

I acknowledge that:

Participation may be subject to NSA’s insurance coverage, where applicable
Coverage is limited and governed by the insurer’s terms
NSA does not provide personal health insurance coverage

I am responsible for maintaining appropriate medical insurance for my child.

7. Code of Conduct & Discipline

I understand that:

My child must follow NSA’s Rules & Regulations and Code of Conduct
Disciplinary action may result from misconduct
NSA decisions related to discipline are final unless otherwise stated

8. Photography, Video & Media Consent

I grant NSA permission to:

Photograph or record my child during NSA activities
Use images or recordings for promotional, educational, or informational purposes

This consent may be withdrawn in writing at any time.

9. Data & Privacy Consent

I consent to the collection and use of personal information for:

Registration and administration
League operations
Communication and safety

This is governed by NSA’s Privacy Policy and complies with PIPEDA (Canada).

10. COVID-19 / Illness Acknowledgement

I acknowledge that:

Participation may involve exposure to communicable illnesses
NSA follows applicable public health guidelines but cannot eliminate all risk

11. Governing Law

This Waiver is governed by the laws of Ontario, Canada.

12. Parent / Guardian Declaration

By completing registration and accepting this Waiver, I confirm that:

I am the parent or legal guardian of the participant
I have read and understood this Waiver in full
I agree on behalf of myself and the minor participant

13. Acceptance Method

This Waiver is accepted by:

Electronic acknowledgement during online registration
Checking the required consent box
Submission of the registration form

Electronic acceptance is legally binding.
